Abstract
Over
the last 60 years, Japanese people have experienced a rapid and drastic change
in lifestyle, including diet. Suspicions have been raised that so-called
'Westernization', characterized by a high-calorie diet and physical inactivity,
is associated with increasing trends in the incidence of cancer of the colon,
liver, pancreas, prostate, and breast, as well as type 2 diabetes.
Epidemiological evidence from our prospective study, the Japan Public Health
Center-based Prospective (JPHC) study, and systematic literature reviews
generally support the idea that factors related to diabetes or insulin
resistance are associated with an increased risk of colon (mostly in men),
liver, and pancreatic cancers. These cancers are inversely associated with
physical activity and coffee consumption, which are known to decrease the risk
of type 2 diabetes. The suggested mechanism of these effects is that insulin
resistance and the resulting chronic hyperinsulinemia and increase in
bioavailable insulin-like growth factor 1 (IGF1) stimulate tumor growth. In
contrast, associations with diabetes are less clear for cancer of the colon in
women, and breast and prostate, which are known to be related to sex hormones.
The effect of insulin resistance or body fat on sex-hormone production and
bioavailability may modify their carcinogenic effect differently from cancers
of the colon in men, and liver and pancreas. In conclusion, there is
substantial evidence to show that cancers of the colon, liver, and pancreas are
associated with insulin resistance, and that these cancers can be prevented by
increasing physical activity, and possibly coffee consumption.

Abstract
A
link between insulin metabolism and colorectal cancer has been hypothesized,
supported by a series of potential physiological mechanisms, and confirmed by a
number of reports in experimental animals. However, the evidence in humans is
limited and mostly indirect.
The
aim of the present report is to analyze whether individuals with a cluster of
metabolic abnormalities associated with abnormalities in insulin metabolism
experience higher mortality for colorectal cancer than those without this
cluster of metabolic abnormalities.
A
total of 21,311 men and 15,991 women 20–69 years of age were followed-up for an
average of 7 years as part of the Risk Factors and Life Expectancy Project, a
pooling of a number of epidemiological studies conducted in Italy.
Our
analyses indicate that participants with high levels of blood glucose and a
cluster of metabolic abnormalities linked to insulin resistance experienced a
significant increased risk of colorectal cancer mortality compared with
participants without the cluster. For the presence of the cluster of metabolic
abnormalities, the calculated hazard ratios and 95% CIs were 2.96 (1.05–8.31)
for men, 2.71 (0.59–12.50) for women, and 2.99 (1.27–7.01) when both sexes were
combined. These associations were independent from the potential confounding
effect of age, drinking of alcoholic beverages, and smoking.
Our
findings are supportive of the hypotheses that glucose metabolism
hyperinsulinemia, insulin resistance, and metabolic abnormalities associated
with it may play a significant role in the etiology of colorectal cancer.

ABSTRACT
Context Obesity, diabetes mellitus,
and glucose intolerance have been associated with increased pancreatic cancer
risk; however, prediagnostic serum insulin concentration has not been evaluated
as a predictor of this malignancy.
Objective To investigate whether
prediagnostic fasting glucose and insulin concentrations and insulin resistance
are associated with subsequent incidence of exocrine pancreatic cancer in a
cohort of male smokers.
Design, Setting, and
Participants A
case-cohort prospective study within the Alpha-Tocopherol, Beta-Carotene Cancer
Prevention Study (1985-1988) cohort of 29 133 male Finnish smokers ages 50
to 69 years. The study included 400 randomly sampled subcohort control
participants and 169 incident pancreatic cancer cases that occurred after the
fifth year of follow-up. All participants were followed up through December
2001 (up to 16.7 years of follow-up).
Main Outcome Measures Incident exocrine
pancreatic cancer identified from the Finnish Cancer Registry.
Results After adjusting for age,
smoking, and body mass index, higher baseline fasting serum concentrations of
glucose, insulin, and insulin resistance were positively associated with
pancreatic cancer. The presence of biochemically defined diabetes mellitus
(glucose, ≥126 mg/dL [≥6.99 mmol/L]) and insulin concentration in the highest
vs lowest quartile both showed a significant 2-fold increased risk (hazard
ratio
, 2.13; 95% confidence interval [CI], 1.04-4.35; and HR, 2.01; 95%
CI, 1.03-3.93; respectively). There were significant interactions for all the
biomarker exposures by follow-up time, such that the positive associations were
stronger among the cases that occurred more than 10 years after baseline
(highest vs lowest quartile: glucose, HR, 2.16; 95% CI, 1.05-4.42; Pfor trend = .02; insulin, HR, 2.90; 95% CI,
1.22-6.92; P for trend = .005;
and insulin resistance, HR, 2.71; 95% CI, 1.19-6.18; P for trend = .006).
Conclusions These results support the
hypothesis that exposure to higher insulin concentrations and insulin
resistance predicts the risk of exocrine pancreatic cancer.
